What a drag. We bought some of these I think in December last year or January this year. I didn't keep UPC's or my receipt from the store.
Actually I was disappointed too, because they went bad before the expiration date, they started to grow this greenish mold on top. I know it wasn't my refrigerator's fault, it's working fine and everything else was staying good. I should have done all the stuff to get my money back, but at the time I just wasn't in the mood and felt ill from other reasons.
Wish I would have kept all the stuff needed so I could enter into this and been able to get my money back.
I'll just stick to Fiber One's yogurt. Fewer calories and I haven't been disappointed with them yet. Yet, the only flavor in the Fiber One Yogurt we like around here is the Strawberry. Don't care for the vanilla or the Key Lime. Wish they had blueberry and Peach.

Just remember that to get the $15 refund, you just need to fill out the online form. No proof of purchase necessary.

Do the receipts need to add up to $30?

My understanding is that for amounts between $15 to $30, you do not need receipts. You do need to list what you purchased or estimate what you can remember. You have to sign the claim form saying that what you wrote down is what you purchased. I have not printed the form so I am assuming that they give a value for each item or let you estimate the value. For amounts over $30, you do need receipts.
